More Than a HIPAA Forms Plugin...
Akashic is a patient-centered, real-time healthcare coordination platform & patient portal embedded within an organization's own WordPress domain.
When we released HIPAA Forms in early 2018, our goal was simple: provide healthcare organizations with a secure and HIPAA-compliant way to collect patient information through their existing WordPress websites.
What we discovered was something much larger.
Over the years, healthcare organizations began using HIPAA Forms for far more than patient intake. It became part of onboarding workflows, patient registration, scheduling processes, internal communication, patient follow-up, and countless other operational tasks.
As organizations pushed the platform further, they repeatedly asked for features such as:
- Patient portals
- Appointment scheduling
- Secure messaging
- Saved forms and drafts
- Staff collaboration tools
- Better role management
- Real-time updates
- Follow-up workflows
- Family and caregiver access
- Better visibility into patient information
The more we listened, the more obvious it became:
The challenge wasn't forms.
The challenge was what happens after the form is submitted.
The Problem We Set Out To Solve
Healthcare information remains fragmented across countless systems, organizations, and portals.
Patients routinely interact with primary care providers, specialists, hospitals, clinics, pharmacies, laboratories, caregivers, and family members. Each organization typically maintains its own records, its own portal, and its own workflows.
As a result, information becomes trapped inside disconnected systems, PDFs, scanned documents, fax transmissions, and records requests.
Even in 2026, patients often wait days or weeks for information that already exists digitally.
That never made much sense to us.
Patients should not have to act as human data couriers between healthcare organizations.
Families should not struggle to stay informed.
Providers should not have to make decisions without complete information.
Most importantly, patients should have meaningful access to the information that directly affects their health.
Why Has It Taken Eight Years?
One question we've been asked repeatedly is:
"If you've been talking about this for years, why has it taken so long?"
The honest answer is that we've tried multiple times over the last eight years.
Each time we reached the same conclusion:
The industry wasn't ready.
The challenge was never simply writing software.
Healthcare lacked several foundational pieces required to build a modern patient-centered platform:
- Mature interoperability standards
- Consistent healthcare data models
- Cloud infrastructure designed specifically for healthcare workloads
- Broad adoption of standardized healthcare data exchange
- Practical ways to coordinate information between organizations
Without those foundations, building another healthcare platform would have simply created another silo.
We weren't interested in contributing to the problem.
Over the last several years, however, the landscape has changed dramatically.
Healthcare interoperability standards such as FHIR have matured. Cloud infrastructure designed specifically for healthcare workloads has become widely available. Standardized healthcare terminologies and data exchange models are becoming increasingly common throughout the industry.
For the first time, the foundation exists to build what we originally envisioned.
Introducing Akashic Health Records
Akashic Health Records is a patient-centered healthcare coordination platform and patient portal embedded directly within an organization's own WordPress environment.
It is designed to help organizations collect information, manage information, coordinate care, and engage patients through a modern real-time experience.
Rather than replacing existing healthcare systems overnight, Akashic is designed to become the coordination layer between patients, families, practitioners, staff, and healthcare organizations.
The goal is simple:
Improve visibility, communication, and coordination while keeping patients closer to their own healthcare information.

Rethinking Forms
One of the biggest lessons we learned from operating HIPAA Forms for nearly a decade is that healthcare organizations spend an enormous amount of time collecting the same information repeatedly.
Patients often complete the same demographic forms, medication lists, allergy information, family history questionnaires, intake paperwork, and registration forms over and over again.
Akashic approaches this differently.
Instead of treating every submission as a standalone document, Akashic stores information as structured healthcare data that becomes part of the patient's profile.
For example:
- Demographics are stored once and reused.
- Medication information updates the patient's medication list.
- Allergies become part of the patient's allergy profile.
- Family history becomes part of the patient's health record.
- Questionnaires can update existing information instead of creating duplicate records.
This reduces repetitive data entry while improving consistency and accuracy.
That does not mean forms disappear.
Organizations will still be able to build custom forms, questionnaires, assessments, intake workflows, consent forms, and operational documents.
The difference is that those forms can now interact with structured healthcare information rather than simply generating another disconnected submission.
Built Around Modern Healthcare Standards
Akashic includes its own integrated healthcare-focused form and questionnaire builder.
Organizations will be able to:
- Create completely custom forms
- Build questionnaires and workflows
- Create patient tasks and assessments
- Start from pre-built templates
- Customize templates to match their workflows
- Reuse forms across multiple organizations and departments
Where appropriate, forms can leverage modern healthcare standards including:
- FHIR
- SNOMED CT
- LOINC
This allows organizations to collect information using standardized healthcare concepts while maintaining the flexibility to build workflows that fit their specific needs.
The goal is not to force organizations into rigid templates.
The goal is to make collected information more useful, more actionable, and more interoperable.
The Evolution of HIPAA Forms
HIPAA Forms was built to securely collect healthcare information.
Akashic Health Records is built to securely collect that information, organize it, standardize it, and make it available where and when it is needed.
Many of the workflows that currently require multiple disconnected forms can instead become part of an ongoing patient profile that evolves over time.
For organizations already using HIPAA Forms, Akashic represents the natural next step in that evolution.

Providers deserve better visibility into the information they need to make decisions.
HIPAA Forms helped us understand these challenges.
Akashic Health Records is our attempt to help solve them.
